Applying for Credit
If you are a business seeking credit, please fill out the credit application in its entirety. You will also need to provide your Tax ID number, signature, printed name and date at the bottom of the form. If you already have a document listing your required information, please send it in. Be sure to sign, print and date this application form and fax it as well.
Government and Federal Agencies
Attention government and Federal government agencies, if you are part of one of the following government organizations and you would like to apply for credit, please fill out the contact information portion of the credit application. All information obtained is confidential and will not, under any circumstances, be released to third parties.

We accept American Express, Visa and MasterCard. Please be sure to provide the exact billing address and telephone number. We will only use credit card information for transaction purposes. Incorrect information could result in a delay of your order being processed or shipped.

Items sold and shipped to destinations in the state of Pennsylvania are subject to sales tax.
Each customer shall be solely responsible for all sales, use, or any other type of taxes, on orders shipped to any states other than Pennsylvania.
